By submitting this form you agree to accept the terms and conditions  set out by Physio Express listed below.

1. You agree to telephone the patient within two hours of receiving a confirmation of their appointment with you the Physiotherapist or clinic.

2. You agree to notify Physio Express of any changes to your contact details, services or any other information that could affect the standard of service a patient will receive.

3. You agree that all invoices submitted to you the physiotherapist or clinic from Physio Express, subject to an attended appointment from a Physio Express patient referral, will be settled within 14 days of receipt. Late payments may result in additional administrative charges, and may apply.

4. You confirm that all information submitted to Physio Express  detailed within this registration form and any further information submitted thereafter by you the physiotherapist or clinic is accurate and true to the best of your knowledge.
